, in addition to clearances. Fill out a form for 1) your current employer, 2) any school entity(ies) where you were previously employed in any capacity and 3) any other employer(s) where you had direct contact with children. Please note that you may have to complete more than one form. Return the completed forms to the Human Resources Office to be distributed. Do not send forms to former employers.
