Be sure to contact the nurse if there is any change in your

child's medical condition,  medication, address, phone numbers

or any new contact information. We want to help your child

by being prepared in the event their is an emergency or event

that requires contacting you. Cell phones are a great way to

communicate. By providing all the information you can, we can
