All students receive vision, height and weight screening
annually. In addition, students in
kindergarten through third grade will have hearing screening done.
Parents will be notified if their child does not pass any of the
screenings. If needed, referral
forms will be sent home for vision and/or hearing evaluations. Remember the
vision screening is a basic examination and not a replacement for a complete
exam by an eye specialist. The
American Optometric Association recommends an eye examination by six months of
age, at three years of age, before first grade and every two years thereafter.
Physical examinations are required for students entering school in kindergarten or first grade and for students in sixth grade. Dental examinations are also required for students entering school in kindergarten or first grade and for students in third grade. Families are encouraged to have their own family doctors and dentists perform these examinations. Private Physician Physical Forms and Dental Examination Forms may be obtained from your school nurse or downloaded from this site. If these forms are not returned to the school, your child will be scheduled for an examination by the school district’s physician and dentist.
